Terms of Use for Hello-Doc Telemedicine (English version):
By using our telemedicine services, you acknowledge that you have read, understood, and agreed to be bound by these Terms of Use and applicable laws and regulations. These Terms apply to all users of the Hello-Doc Telemedicine platform provided by Bataan General Hospital and Medical Center.
Eligibility:
You must be at least 18 years old or have the legal capacity to form a binding contract in your jurisdiction to use this service, if the user is not a Filipino citizen. Minors may use the service only under the supervision of a parent or guardian.
Accurate Information:
You agree to provide accurate, current, and complete information when creating an account or submitting health-related information. Submitting false or misleading information or using another person’s identity to gain benefits is strictly prohibited and may result in termination of access to the service. Bataan General Hospital and Medical Center reserves the right to suspend or terminate access for users who violate this condition.

We do not guarantee any medical outcomes, and our healthcare providers’ advice is based solely on the information you provide. Bataan General Hospital and Medical Center makes no representations or warranties regarding the medical advice or consultation provided through the platform.
Intellectual Property:
All content, including text, graphics, logos, images, and software used in the Hello-Doc Telemedicine platform, is the property of Bataan General Hospital and Medical Center and is protected by intellectual property laws. You may not use, modify, or distribute any content without explicit permission from the company. Unauthorized use or reproduction of any content may result in legal action.
Termination of Service We reserve the right to suspend or terminate your access to the service at any time, for any reason, including but not limited to violations of these Terms of Use or engaging in fraudulent, illegal, or harmful activities. Mga Tuntunin ng Paggamit para sa Hello-Doc Telemedicine (Tagalog version):
Sa paggamit ng aming mga serbisyo sa telemedicine, kinikilala mo na nabasa, naintindihan, at sumasang-ayon kang sumunod sa mga Tuntunin ng Paggamit na ito at mga naaangkop na batas at regulasyon. Ang mga Tuntunin ng Paggamit na ito ay sumasaklaw sa lahat ng mga gumagamit ng Hello-Doc Telemedicine platform ng Bataan General Hospital and Medical Center.
Kwalipikasyon sa Paggamit:
Ang gagamit ay dapat 18 taong gulang na o may legal na kapasidad na sumangayon sa kasunduan upang magamit ang serbisyong ito, kung ang gagamit ay hindi Filipino. Ang mga menor de edad ay maaari lamang gumamit ng serbisyong ito kung sila ay nasa patnubay at gabay ng isang magulang o tagapangalaga.
Tamang Impormasyon:
Sumasang-ayon kang magbigay ng wasto, napapanahon, at kumpletong detalye kapag gumagawa ng account o nagsusumite ng impormasyon ukol sa kalusugan. Ang pagsusumite ng maling impormasyon o paggamit ng detalye ng ibang tao upang makakuha ng benepisyo ay mahigpit na ipinagbabawal at maaaring magresulta sa pagkansela ng akses sa serbisyo. Ang Bataan General Hospital and Medical Center ay may karapatang suspindihin o tapusin ang akses ng mga gumagamit ng serbisyo na lumabag sa kondisyong ito.

Privacy at Proteksyon ng Datos:
Pinahahalagahan namin ang iyong privacy. Sa paggamit ng aming mga serbisyo, sumasang-ayon kang kolektahin, gamitin, at ibahagi ang iyong personal at impormasyong pangkalusugan ayon sa nakasaad sa aming Privacy Policy. Kami ay dedikado sa pag-iingat ng iyong personal na impormasyon at sa pagsunod sa mga naaangkop na batas ukol sa proteksyon ng mga datos. Limitasyon sa pananagutang medikal:
Ang mga serbisyo ng Hello-Doc Telemedicine ay hindi kapalit ng personal na konsultasyong medikal. Ang aming platform sa telemedicine ay nagpapadali lamang ng mga online na konsultasyon kasama ang mga healthcare providers, ngunit nararapat kang pumunta ng personal kung emergency ang iyong kaso o kung kritikal ang isyu sa iyong kalusugan. Hindi kami nagbibigay garantiya sa anumang resultang medikal, at ang mga payo’t konsultasyon mula sa aming mga healthcare providers ay batay sa impormasyong iyong ibinibigay. Wala ring aakuin o ipagkakaloob na mga warranty ang Bataan General Hospital and Medical Center ukol sa payong medikal o konsultasyon na ibinibigay sa pamamagitan ng platform na ito.
Intellectual Property:
Ang lahat ng nilalaman, kabilang ang teksto, graphics, logo, larawan, at software na ginamit sa Hello-Doc Telemedicine platform, ay pag-aari ng Bataan General Hospital and Medical Center at protektado ng mga batas ukol sa intellectual property. Hindi maaaring gamitin, baguhin, o ipamahagi ang anumang nilalaman nang walang tahasang pahintulot mula sa kumpanya. Ang hindi awtorisadong paggamit o pag-gaya ng anumang nilalaman ay maaaring magresulta sa legal na aksyon.
Pagtatapos ng Serbisyo:
May karapatan ang Bataan General Hospital and Medical Center na suspindihin o tanggaling ang iyong akses sa serbisyo anumang oras, sa anumang dahilan, kabilang ngunit hindi limitado sa mga paglabag sa mga Tuntunin ng Paggamit na ito o pakikilahok sa mga mapanlinlang, iligal, o mapanirang gawain. Aabisuhan ka tungkol sa anumang hakbang laban sa iyo at maaaring bigyan ka ng pagkakataong maitama ang iyong ang paglabag ayon sa pagpapasya ng Bataan General Hospital and Medical Center.
Pagbabago sa Mga Tuntunin ng Paggamit:
Maaaring i-update o baguhin ng Bataan General Hospital and Medical Center ang mga Tuntunin ng Paggamit na ito anumang oras. Ang anumang pagbabago ay ipo-post sa pahinang ito, kasama ang petsa kung kailan ito naitala. Sa patuloy na paggamit ng serbisyo ng Telemedicine, ikaw ay sumasang-ayong sumunod sa mga binagong tuntunin.
